The Youth in Transition Worker (YITW) program provides a variety of support services to youth as they leave the care of the CAS and transition to adult services. The YITW program has been developed in response to advocacy by youth who have left the care of the CAS and who have faced challenges sourcing services and programs independently.
YITW program services are available to youth 16-24 years of age.

The YITW helps youth plan and make decisions regarding:
- Housing
- Education
- Health care
- Life skills
- Employment
- Personal goals & well being
